Who is eligible?

The program is available to medical benefit enrollees over the age of 18 who meet the following criteria:

Autoimmune:

  • Have a diagnosis of Psoriasis, Psoriatic Arthritis, Rheumatoid Arthritis or Ankylosing Spondylitis
  • Are on a biologic medication
  • Live in Ohio

Migraine:

  • Have a migraine condition (can be evaluated by AndHealth)
