Copay varies by type of Medicare or Medicare Advantage plan. Typically, a specialist copay is charged for specialist visits, and a primary care/office copay for clinical pharmacist or NP/MD visits. Health coach and patient navigator are provided with no copay.

Costs according to your health plan. A financial assistance program from AndHealth may be available after secondary insurance and drug coupons are applied. Talk with your AndHealth provider team for more information.

One of our Care Navigators will work with you to find primary care and specialist care on a sliding fee scale, even if you do not have an insurance plan right now. Additionally, they will help you find other assistance programs that can help with housing, transportation, groceries, and utilities

Included in your care plan at no cost:

  • Health coach visits to support medication routine and lifestyle changes
  • Patient navigator support for medication prior authorizations, copays, provider scheduling, and/or transportation

  • In-home visit from a mobile phlebotomist to draw labs

Included at no cost, if part of your care plan:

Costs billed according to your health plan:

Your AndHealth care team includes patient navigators who work with you & your insurance company for the lowest out-of-pocket treatment costs

We understand that chronic condition management is very expensive for diagnosed people and their families. Your care team will always help you know the cost of treatment and work to make sure your OOP (out-of-pocket) is as low as possible for the treatment and medication you need.

We are in-network with Medicaid, Medicare, TRICARE, and most commercial (PPO, HMO, EPO, POS) health plans in Ohio and Indiana. Through our partnerships with Community Health Centers, we will help you schedule a provider appointment, even if you are uninsured at this time.
