We love partnering with providers around the community! Refer your patient to AndHealth for care. We provide convenient access to patient-centered care for all patients, including those covered by Medicaid and Medicare.

Appointment requests can be made via fax or by calling our patient onboarding line. When possible, please provide any relevant patient notes, past labs, and treatment history. We recommend sending referrals via fax when possible, to ensure we receive the most complete patient records.
