Lower Lights Health Selected as the 2025 OCHIN Impact Award Winner for Transforming Patient Health and Well-Being Through Its Partnership With AndHealth

COLUMBUS, OH – Lower Lights Health (LLCHC) was honored this week as the 2025 OCHIN Impact Award Winner for Transforming Patient Health and Well-being at the 2025 OCHIN Member Learning Forum in Nashville, TN.
The OCHIN Learning Forum is an annual healthcare enrichment event that features the annual Impact Awards, which recognize four healthcare leaders that have used tools and insights in transformative ways to improve the health and well-being of their patients, providers, and communities. Lower Lights Health was selected as a 2025 winner for its partnership with AndHealth to offer specialty care and specialty pharmacy services to their patients. The partnership leverages OCHIN Epic to create a referral and provider communication pathway between the two organizations.
Lower Lights Health has been at the forefront of providing specialty care in the Community Health Center (CHC) setting by first partnering with AndHealth to provide virtual and onsite care for autoimmune conditions and migraine. They’ve since expanded their partnership to also include onsite specialty care for rheumatology, dermatology, and select neurology, onsite specialty pharmacy and contract pharmacy support, and establishing primary care referral connections between health systems and LLCHC. Together, Lower Lights Health and AndHealth are also expanding care to new therapeutic areas such as Infectious Disease, Endocrinology, Immunology, and Gastroenterology.
“Adding specialty care to our services has allowed us to support patients in new and critical ways. Historically patients have waited months to see specialists and many of our patients have faced social drivers of health that prevented them from receiving the care they need,” said Tracy Cloud, CEO of Lower Lights Health. “Our patients can now be seen in less than two weeks, onsite or virtually, and care and medications are available on a sliding fee scale. Patients have access to things like health coaching, nutritional food, sleep and stress management support – all included within this whole-person approach to care.”
AndHealth team members attended the awards ceremony in Nashville to celebrate their collaborator’s recognition.
