Consent to Communications

By accepting this consent to communications (“Consent”), you agree that AndHealth Providers, P.C. and AndHealth LLC (collectively, “AndHealth”) may contact you via electronic messaging, email, phone, text, or mail, including communications pertaining to your care and your health, which may include your personally identifiable information (“PII”) and/or your protected health information (“PHI”), as well as promotional services about new offerings provided by AndHealth.

You understand that communication via email, text messages, and other electronic means may not be secure and that your PII and/or PHI could be viewed by unintended persons. By accepting this Consent, you acknowledge these risks and agree to the exchange of such communications, to and from AndHealth, via these electronic means. You also agree to update your contact information as necessary to ensure accuracy.

Normal text messaging and data usage rates will apply to messages you receive from AndHealth.

If you later decide that you do not want to receive certain future communications electronically, please send an email to [email protected], or a letter, with a copy also sent via email, to AndHealth at 2 Miranova Pl, Suite 500, Columbus, OH 43215. You may also opt out of certain electronic communications through your account or by following the unsubscribe instructions in any communication you receive from AndHealth. Your withdrawal of consent will be effective within a reasonable time after we receive your withdrawal notice described above.
