Who is eligible?

Anyone covered by their employer’s health benefit through the Ohio Chamber Health Benefit Program meeting the following criteria:

• Have a diagnosis of Psoriasis, Psoriatic Arthritis, Rheumatoid Arthritis, or Ankylosing Spondylitis
• Are taking a biologic medication
• Are over the age of 18
